Nu Outlook provides SOAR-informed benefits assistance as part of a structured stabilization pathway.
SOAR (SSI/SSDI Outreach, Access, and Recovery) is an evidence-based model developed by the Substance Abuse and Mental Health Services Administration (SAMHSA) to improve access to Social Security disability benefits for eligible individuals who are experiencing homelessness or at risk of homelessness and who have disabling physical or behavioral health conditions.
In Ohio, SOAR coordination is supported through the Coalition on Homelessness and Housing in Ohio (COHHIO).
The model strengthens application quality through:
• Comprehensive medical and behavioral health documentation
• Functional narrative development aligned with SSA criteria
• Coordinated communication with providers
• Procedural follow-through to reduce avoidable denials
SOAR-informed assistance is delivered by a Certified SOAR Representative and integrated into a broader stabilization plan.
Support may include:
• SSI/SSDI application preparation
• Medical documentation coordination
• Functional summary development
• Ongoing follow-up during SSA review
• Assistance responding to SSA requests

SOAR-informed assistance may be appropriate when a physical or behavioral health condition significantly limits daily functioning and the ability to sustain age-appropriate or work-related activities.
Important Note
Not all individuals experiencing hardship qualify for SSI or SSDI. A structured eligibility screening is conducted before proceeding with SOAR-informed application support.
A child may be appropriate for screening when:
• An Individualized Education Program (IEP) or 504 Plan is in place due to functional limitations
• There are documented developmental, cognitive, emotional, or behavioral health diagnoses
• The child experiences significant difficulty interacting with peers or authority figures
• School attendance, concentration, or age-appropriate activities are substantially affected
• Medical or mental health providers have identified ongoing impairments
Eligibility is determined under SSA’s child disability standards, which assess functional limitations across multiple domains.
An adult may be appropriate for screening when:
• A diagnosed physical or behavioral health condition limits sustained employment
• There is a history of trauma impacting daily functioning
• Justice involvement has intersected with untreated or ongoing health conditions
• Housing instability is connected to disabling health-related barriers
• Medical providers document long-term functional limitations
